Medical Requirements For AdmissionPhysical Examinations of Students1. A current physical examination is required for all new students. A current physical examination is required for all students when entering grades one, four, seven, and ten. 2. This exam must be dated within 6 months from date of the entrance to school, and signed by a physician. 3. The medical examination report is to be kept in the health office and is part of the cumulative record folder. Health forms can be obtained from the school office. Medications State regulations do not allow for any medications other than Epi pens to be administered at school by school personnel, other than the school nurse. Medication prescribed or non prescribed is not allowed in the classroom. The only medications allowed to be self administered by students are Epi pen, inhalers, and insulin. Please note if your child is in need of medication during school hours, a parent will need to come to administer it. Please DO NOT send medication with students in their lunch box. Vaccine Information Up-to-date immunization records must be kept and supplied with a student's application.
NOTE: In accordance with state law, failure to present documentation of immunizations by October 1 will result in exclusion from school until compliance is reached. The General Laws of Massachusetts, Chapter 76: Section 15. |
